1. Platform Role Disclaimer

AffilaForm is a lead generation platform that enables vendors to create white-labeled forms and affiliates to promote them in exchange for potential commissions.

We are not a party to any payment, contract, or business relationship between vendors and affiliates. AffilaForm does not process payments, enforce commission terms, or mediate disputes between users.

All leads, payouts, and commissions are the sole responsibility of the vendor.

2. Vendor Responsibilities

By using AffilaForm, vendors agree to:

• Pay affiliates fairly and promptly based on the payout terms displayed.
• Maintain accurate records and communication with their affiliates.
• Honor the integrity of the lead submissions and avoid disputes in bad faith.

Failure to meet these standards may result in:

• Removal from the platform.
• Loss of access to affiliate tools.
• Permanent ban at our discretion.
